Provides managed long-term care services to frail elders (age 60 and older) and people age 18–59 with physical, intellectual and/or developmental disabilities. Provides support in caring for their health and doing activities of daily living so they can be as independent as possible. Family Care members must qualify financially and functionally for the program.
