Supports young adults who are experiencing emotional and behavioral challenges to successfully transition into adulthood. Transitional coordinators can assist with housing, employment, school, health and being safe. To be eligible the young adult must be Medicaid eligible, have a mental health diagnosis or behavioral/emotional challenges and be between the ages of 17-24 years old.
